Chesapeake Integrated Behavioral Healthcare is seeking two skilled and empathetic. Licensed Practical Nurses (LPN) to provide exceptional care to our Psychiatric Services outpatient clinic.   • Administration of Injections
  • Provide medication education, crisis intervention and resource information.
  • Phone call triage for medication refills, form requests, crisis intervention, etc.
  • Medication prior authorizations.
  • Respond to crisis situations which may include Emergency Services contact and participation in the TDO and/or ECO process.
  • Care coordination through collaboration with a multidisciplinary team (registered nurses, prescribers, case managers, therapists, etc.)
  • Provide documentation in the electronic health record system.
  • Completion of primary care screenings (height, weight, auto-populated Body Mass index (BMI) and vital signs.
  • Complete referrals, tardive dyskinesia screenings, as well as fall risk assessments.
  • Performs other duties as assigned.
