Job Description
- Administration of Injections
- Provide medication education, crisis intervention and resource information.
- Phone call triage for medication refills, form requests, crisis intervention, etc.
- Medication prior authorizations.
- Respond to crisis situations which may include Emergency Services contact and participation in the TDO and/or ECO process.
- Care coordination through collaboration with a multidisciplinary team (registered nurses, prescribers, case managers, therapists, etc.)
- Provide documentation in the electronic health record system.
- Completion of primary care screenings (height, weight, auto-populated Body Mass index (BMI) and vital signs.
- Complete referrals, tardive dyskinesia screenings, as well as fall risk assessments.
- Performs other duties as assigned.
