Job Summary:Pharmacists at Kaiser Permanente ensure that patients receive safe, effective, and optimal medication therapy while providing expertise and information to empower patients and prescribers to make informed choices in medication therapy. Pharmacists ensure that pharmacy care is coordinated with the patient care plan.
Infusion pharmacists service the Hematology/Oncology Infusion Clinic, Advanced Urgent Care, and Ambulatory Surgery Center with sterile compounding and infusion needs.
Essential Responsibilities:
- Ensures that patients receive safe, effective, and optimal medication therapy by: Interpreting prescription orders; counseling patients using clear and effective communication skills and providing written information as necessary; ensuring patient safety by identifying opportunities to prevent unusual occurrences; consistently performing safety checks when dispensing medications; taking appropriate steps to resolve and report unusual occurrences.
- Provides medication expertise and information that empowers patients and prescribers to make informed choices in medication therapy by: by using appropriate evidence-based medicine references to form recommendations to patients and providers; clearly communicating rationale around medication therapy selection; evaluating clinical appropriateness of medication therapy; staying current on clinical initiatives and priorities
- Ensure efficient and effective pharmacy process workflow and cash handling by: supervising pharmacy assistants, technicians, interns and externs in all aspects of daily pharmacy workflow including the appropriate processing and releasing of prescriptions and customer service; ensuring the proper and safe storage of medications and keep accurate records of such; having working knowledge and competency with cashiering processes and procedures; adhering to Kaiser Permanente Cash Security and Compliance standards
- Ensures practice adherence and compliance of USP <797> and USP <800> guidelines for sterile compounding of hazardous and non-hazardous drugs.
- Collaborates with clinic staff including providers and nurses to provide oncology and non-oncology medication expertise of infusion medications according to diagnosis and plan coverage criteria.
- Collaborates with urgent care and ambulatory surgery center providers and nurses to provide medication expertise and dispensing of medications through automated dispensing machines (Pyxis).
- Builds, initiates, modifies, and maintains oncology and non-oncology treatment and therapy plans in collaboration with providers.
Basic Qualifications: Experience
- Minimum one (1) year of experience working in a pharmacy practice environment using current technology.
- Bachelors degree in pharmacy or PharmD required.
License, Certification, Registration
- Pharmacist License (Washington) required at hire
- Ability to work effectively in a team environment.
- Ability to prioritize workload effectively.
- Verbal and written communication skills.
- Customer service skills.
- Ability to work quickly and efficiently.
- Ability to evaluate clinical appropriateness of medication therapy.
- Knowledge of major medication therapy, drug interactions, and disease states.

- Ability to work effectively in a team environment.
Preferred Qualifications:
- Three (3) years of experience working in a pharmacy practice environment using current technology.
- Experience interacting with an interdisciplinary team including physicians.
- PGY1 training in Acute Care
- PGY2 training in Oncology
- Board Certification in Oncology
- Experience working in infusion or sterile compounding
