Job Overview
We are seeking a compassionate Advanced Practice Provider (NP or PA) to deliver high-quality, patient-centered care within a PACE (Program of All-Inclusive Care for the Elderly) model. This role focuses on managing complex medical needs for an aging population through comprehensive assessments, care planning, and close collaboration with an interdisciplinary team.

Requirements to apply
- Licensed Nurse Practitioner (NP/APRN) or Physician Assistant (PA); MD/DO considered
- Active New Jersey license
- DEA and CDS required
- BLS certification (within 90 days of hire)
- Minimum 2 years of direct patient care experience preferred
- Experience with geriatric or frail populations preferred
- Board Certification in Geriatrics preferred
