Location: MetroHealth Old Brooklyn Campus

Biweekly Hours: 80.00

Shift: 8:30am- 5pm

Summary:


Initiates one-on-one dialog with potential referral sources and maintains positive relationships with current referral sources. Design strategies to increase patient admissions that leverage MetroHealth Rehabilitation Institute's core competence in the area of rehabilitating medically complex patients. Identifies patients who meet rehabilitation program criteria and will benefit from rehabilitation services. Educates patients, family members, referral sources, and external payers about the effective/efficient utilization of rehabilitation services. Determines the medical stability/readiness of potential clients through on-site/telephone evaluations, chart reviews, and interactions with physicians/nurses/therapists involved in the patient's care. Obtains complete/accurate patient information and payer authorization(s) for acute rehabilitation services. Completes all required pre-admission screening documentation to determine that services are appropriate/reasonable/necessary. Coordinates the client's transition from the referring facility to the MetroHealth Rehabilitation Institute. Recommends alternatives for candidates who are denied admission. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.

Qualifications:

Qualifications: Required: Possesses one of the following: • Graduate of an accredited school of professional nursing and current Ohio RN license. • Graduate of an accredited school of Physical or Occupational Therapy and current Ohio PT or OT license. • Graduate of an approved school of practical nursing and current Ohio LPN license. • Bachelor's Degree in social sciences or related health care field and recent rehab admission experience. Excellent marketing skills. Excellent interpersonal, written, and verbal communication skills. Preferred: 2 years of degree-related clinical experience in rehabilitation. Demonstrated proficiency with computers, software, and a variety of office equipment. Demonstrated familiarity with hospital admissions procedures. Possess a current certification from an approved issuing body (i.e. Rehabilitation, Trauma, etc.). Familiarity with third-party reimbursement processes and payer regulations, including for managed care. Physical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. Ability to communicate in face-to-face, phone, email, and other communications.
