Job Description

The Care Coordinator is responsible for assisting in the identification and scheduling of patients that have care needs and pending consults.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provides telephonic outreach and engagement to patients to ensure coordination of care, schedule appointments and necessary follow-up.
  • Ensures provider is aware of what needs to be addressed during patients’ appointments.
  • Contacts patients who did not schedule or were a no-show for consult appointment to identify those patients that are still planning to see consulting provider.
  • Scrubs schedules to identify patients that have gaps of care.
  • Schedules patients with appropriate provider, site, and time.
  • Audits and updates patient electronic health records to ensure all necessary information is available.
  • Identifies needed consult notes and contacts appropriate provider to obtain and close consult.
  • Schedule consults based on guidelines.
  • Maintains regular and predictable attendance.
  • Other duties as assigned.

QUALIFICATIONS

Minimum Qualifications:

  • High school diploma or equivalent

Preferred Qualifications:

  • Graduate of a medical assistant program with at least 2 years of related work experience in Family Medicine or Internal Medicine
  • 1 year of customer service experience.
