Care Coordinator - CBHC
The Care Coordinator serves as an integral member of the interdisciplinary Adult Team (primary mental health challenges), providing person-centered and recovery-oriented case and care management support to assigned persons served, including but not limited to outreach and engagement, help persons served to identify goals and to assist in developing an individual service plan.
The Care Coordinator will collaborate with persons served, their treatment team, other providers, identified family and community support, involved state agencies and others, assisting them with navigating resources and connection to services in the community that will continue to improve and maintain their health and quality of life.
In this vital role, the Care Coordinator will:
- Functions as the primary care coordinator for assigned persons served including conducting face-to-face, telephonic and electronic outreach with persons served and maintaining ongoing communication
- Conducts outreach and engagement activities with persons served to find resources to meet needs such as housing, primary care providers, and employment in coordination with the interdisciplinary team
- Assists person served with navigating through all systems of care, provides referrals to appropriate services and supports, coaches and assists them in attaining health and wellness goals
- Coordinates with all members of the CBHC team to ensure all aspects of the CBHC services are coordinated, provided and reviewed as needed and in accordance with program protocols and policies
Schedule: Full Time, 40 hours
- Hybrid schedule available - will include one in-person weekday evening and one in-person weekend day
Pay Rate: $22.27/hourly with differentials!
- additional $1 for CPS
- additional $2 for CARC

Required Skills
- Ability to use a computer for word processing, EHR and email/Outlook Calendar, and the Internet for resource searches persons served
- Must be able to establish a rapport and work effectively with a diverse multicultural group of persons served and program staff members.
- Regular access to reliable transportation is required in order to attend required trainings, meetings and/or fulfill other responsibilities of this role.
Required Experience
Candidates must have one of the following:
- High School Diploma/GED with one year of professional or lived experience
- Associate’s degree in a behavioral health or social service field with one year of professional or lived experience
- Bachelor’s Degree (preferred but not required)
- One year of work experience in very similar role
- Lived experience with mental health or substance use challenges strongly preferred
- Must have experience navigating community resources serving persons with behavioral health challenges and/or struggling with substance misuse
- Must embrace person-centered planning philosophy
