Summary:

Provides general clerical, secretarial and organizational support to the Manager of Care Management, to the departmental staff, and to the department. 


Responsibilities:

1. Prioritize Daily Activities. •Participate in daily Care Management Touchpoint -Care Managers (CM), Social Workers (SW), and Point of Entry Utilization Managers (POE UM) •Discuss referred cases, including, patient status, barriers to discharge, payer communication, escalations and support needs •Review insurance coverage updates on all appropriate patients, as assigned •Regularly evaluate work list (hourly) and re-prioritize items for the day -Review patient census for each service with CM, SW, and POE UM, prioritizing communication needs -Review discharge planning priorities.

2. Assist in Facilitating Discharge Planning.

3. Assist in Utilization Management Process.

4. Complete FL2 for Long Term Care and Pre-Admission Screening and Resident Review (PASRR), as assigned.

5. Collaborates and communicates with departmental and system-wide staff, supervisory and management personnel, with various outside representatives, and patients/families to facilitate timely and effective service.

6. Responsible for the delivery of Regulatory Notices including the Medicare Important Message, Medicare Outpatient Observation Notice and the Medicare Change of Status Notice, assisting in facilitating discharge planning, and assisting in utilization management processes.

7. Performs other duties as needed. 




Other Information:

Education Requirements:
● High School Diploma or equivalent.

· Associates or Bachelor’s Degree preferred

 

Licensure/Certification Requirements:

· Certified Nursing Assistant/Certified Medical Assistant

Professional Experience Requirements:
● 3 or more years of General - Secretary experience

· Experience in a hospital setting preferred
