Job Description

Mountain Park Health Center is looking for Clinical Pharmacists to join our well-established and accredited group of clinical pharmacists who provide high-quality patient care and services to patients at one of our many outpatient clinics. As a member of the integrated health care team, the Clinical Pharmacist collaborates with the medical provider to provide specialized pharmaceutical services to patients to improve drug usage and therapeutic outcomes for Mountain Park Health Center. The position facilitates medication management through medication reconciliation, evaluation of adherence, effectiveness, and toxicity, and is a resource to providers and members of the care team on issues concerning drug therapy, and side effects, as well as assisting in appropriate doses.

Essential Functions

  • Conducts and evaluates medication histories, assesses compliance, and suggests modifications for optimal drug product selection in order to achieve desired outcomes.
  • Individualizes medication regimens using sound principles and understanding of drug absorption, distribution, metabolism, and elimination to establish and improve optimum patient benefit. Provides information and advice regarding drug interactions, side effects, dosage, and proper medication storage.
  • Prevents, detects, and resolves medication-related problems.
  • Makes evidence-based, patient-centered drug therapy recommendations as part of the Integrated Care Team.
  • Follows pharmacy protocols and fulfills medication refills from other pharmacies. Participate in pharmacy dispensing activities.
  • Utilizes clinical pharmacy skills including contraindication and duplicate therapy checks, assessing, and formulating recommendations to clinicians ensuring quality, and economical drug therapy.
  • Performs chart reviews to facilitate proactive problem solving, productivity, and teamwork.
  • Provides concise, applicable, comprehensive, and timely responses to medication queries from patients and providers.
  • Coordinates with Mountain Park Health Center Pharmacy Staff and participates in provider and pharmacy student education.
  • Supports the health care team’s efforts to provide patient-centered care as part of the integrated healthcare team, educate patients on health maintenance activities, and facilitate referral to other integrated services such as behavioral health and dieticians.
  • Maintains regular and predictable attendance.
  • Travels to multiple locations as assigned which may include a location change during shift.
  • Performs other duties as assigned.

Qualifications:

Minimum Qualifications:

  • At least 1 year of experience in a clinical setting or completion of an accredited clinical residency program.
  • Arizona Pharmacist License.
  • Immunization certification.

Preferred Qualifications:

  • Certified Diabetes Educator (CDE) or Board-Certified Advanced Diabetes Management (BC-ADM) Certification Residency.
  • Board Certified Pharmacotherapy Specialist (BCPS).
