Job Description
Family Medicine Nurse Practitioners (FNP) will provide high-quality medical care to a wide variety of patients in a comprehensive, quality oriented, and supportive manner for Mountain Park Health Center. Your role will encompass a broad range of primary care services, focusing on preventive care, health maintenance, the management of acute and chronic medical conditions and follow-up services to patients under your care.
Essential Functions:
- Provides direct patient care by obtaining medical history, performing appropriate physical examination, and creating an assessment and plan that may include ordering and interpreting labs, ordering imaging studies and developing treatment plan including referring to specialty care when needed.
- Administers or prescribes treatments and medications.
- Follows up and monitors patient’s progress throughout treatment plan.
- Documents the evaluation and plan of care.
- Provides patient education on a variety of health topics.
- Collaborates and consults with other in house allied health professionals to ensure coordinated patient care.
- Maintains regular and predictable attendance.
- Performs other duties as assigned.
Position Qualifications:
Minimum Qualifications:
- Approved Nurse Practitioner education program, experience in clinical area of specialty.
- Valid Arizona Registered Nurse License.
- Valid Arizona Nurse Practitioner certificate.
- Valid CPR certificate.
Preferred Qualifications:
- 1-year relevant experience in Family Medicine or Geriatrics
