Job Description
Nurse Practitioner (NP) will provide comprehensive care for adults with obesity, diabetes, and related metabolic conditions, working closely with a clinical pharmacist, dietitian, and behavioral health specialist.
Essential Functions:
- Conduct comprehensive assessments, develop individualized treatment plans, and monitor progress for patients with obesity and diabetes.
- Manage GLP-1 and other anti-obesity/diabetes medications in collaboration with the clinical pharmacist, ensuring safe initiation, titration, and monitoring.
- Provide education and counseling on nutrition, lifestyle changes, and self-management strategies in partnership with the dietitian and behaviorist.
- Monitor clinical outcomes, address side effects, and adjust treatment plans based on patient response and evidence-based guidelines.
- Participate in team huddles and interdisciplinary case reviews to ensure coordinated, high-quality, patient-centered care.
- Maintain accurate, timely clinical documentation and contribute to quality improvement initiatives within the clinic.
- Maintains regular and predictable attendance.
- Performs other duties as assigned.
Position Qualifications:
Minimum Qualifications:
- Approved Nurse Practitioner education program, experience in the clinical area of specialty.
- Valid Arizona R.N. License.
- Valid Arizona Nurse Practitioner certificate.
- Valid CPR certificate.
Preferred Qualifications:
- 1 year of experience in Family Medicine or Geriatrics.
- Strong interest in obesity medicine, diabetes care, and chronic disease management.
