Practice Setting: Office with rotating call obligations with the hospital

Reason for Coverage: Provider resignation

Dates of Coverage: Start date before 11/14

Schedule: M-F 8:30-4:30

Call Ratio: 1:3

Average Number of Scheduled Patients Per Day: Variable based off of scheduled visits-generally ranging from 10-15 depending on visit type and call volume

Number of Physicians in Practice: Currently 3

Onsite Lab?: Yes

Onsite Ultrasound? Yes

Nursery Level (I, II, III): III

Documentation System/EMR: Epic
