Personal Service Coordinator

About the Role
The Personal Service Coordinator (PSC) I/II is responsible for coordinating the needs of clients and families assigned to their caseload. The role ensures individualized care planning, facilitates access to community resources, and monitors the effectiveness of services. PSCs provide professional support through client contact, guidance, and instruction, maintaining documentation and compliance with program and contractual requirements.
Responsibilities
Personal Service Coordinator I
Conduct intake and complete comprehensive evaluation of clients' life situations
Prepare individualized plans of care in collaboration with clients
Facilitate access to community resources and monitor participation in services
Provide professional care through regular client contact, including home visits
Maintain accurate daily records, progress notes, and billing forms per program and Short Doyle/Medi-Cal standards
Develop and maintain professional working relationships with agencies and service providers
Adhere to HIPAA standards, ethical standards, and professional boundaries
Transport clients as necessary using personal vehicle and follow program guidelines
Participate in after-hours phone response duties as assigned
Perform other duties as required to support department and program goals
Personal Service Coordinator II
All duties of PSC I
May conduct 5585/5150 evaluations and possess designation under LPS /5150 evaluations and possess designation
Serve as an advanced resource within scope of practice for case management
Recovery Requirements
Provide the highest quality customer care to clients and families
Support clients' steps toward recovery and wellness
Celebrate diversity and create inclusive organizational culture
Utilize industry best practices to ensure optimal client outcomes
Promote professional growth and continuous improvement
Education and Licensing Requirements
PSC I:
Bachelor's degree in Human Services or related field with 1 year of related experience
Eligible to claim for mental health services (except psychotherapy) within training and scope of practice
PSC II:
Bachelor's degree in Human Services or related field with 4 years of related experience
Up to 2 years graduate education may substitute for experience on a year-for-year basis
Up to 2 years post-associate clinical experience may substitute for education requirement
Eligible to claim for mental health services (except psychotherapy) within training and scope of practice
