OBJECTIVE/PURPOSE
Under the direction of the Family Services Program Manager, the Family Services Program Coordinator provides administrative and clinical supervision to staff working with clients with intellectual disabilities who live with their families. The Coordinator will assess, plan, implement, monitor, and evaluate the needs of clients and respond accordingly. The Coordinator ensures that the services provided through Family Services meets the needs of the clients, ensures that services are delivered with dignity and respect, and meet regulatory requirements.
Completes all tasks necessary for the implementation of the Bridge program, and Family Managed Respite (FMR) funding.
MAJOR RESPONSIBILITIES
1.0 PROGRAM DEVELOPMENT AND SUPERVISION
Provides administrative and clinical support to Case Managers and the Program Manager. Monitors quality of services provided. Assists Case Managers with establishing meaningful treatment plan (ISA) goals. In conjunction with the Program Manager and Human Resources, trains, motivates, and supports staff. Conducts periodic inservice training, as appropriate. Assists Case Managers in developing creative programs. May maintain an active caseload of Families clients. Participates as a member of the DS Emergency On-Call System. Encourages and facilitates skill development of staff. Ensures program services and supports are being carried out consistently.
2.0 QUALITY ASSURANCE AND COMPLIANCE
With the Program Manager, ensures that programs and documentation meet regulatory compliance. Facilitates programmatic changes to meet the needs of clients. Reviews data and provides feedback to Case Managers and the individual's team. Reviews and approves staff's required agency documentation, as appropriate. Assists Case Managers in ensuring that waiver budgets match the services provided to the consumer. May provide case management services as assigned. Will work up to performing QDDP duties as qualifications allow.
3.0 NETWORKING AND CONSULTATION / EDUCATION
Serves on agency committees, as assigned or appropriate. Assists Case Managers in the coordination of services, as requested. Participates in school-based meetings to support the transition of students to adult services. Participates in Coordinated Services Plan (CSP) meetings. Completes Children's Personal Care assessments and ensures case managers are trained to complete as needed. Provides assigned training as requested or appropriate. Represents the division on statewide task forces as assigned. May contribute to community awareness through the media or speaking engagements. Maximizes the division's reputation.
4.0 INFORMATION MANAGEMENT, RECORD KEEPING, AND FISCAL MONITORING
Reviews various clinical and administrative forms for billing, management information, and clinical purposes. Prepares reports, as requested. Reviews management and outcome reports completed by managers. Reviews waiver budgets as appropriate and advises Division Director of necessary changes. Ensures the timely completion of documentation from Case Managers.
* In addition to the above, performs other duties as assigned.
EDUCATIONAL REQUIREMENTS
Bachelors Degree preferred but, relevant experience will be considered in lieu of a degree. Familiarity with all programs in Microsoft Office, Outlook, and Electronic Medical Record helpful. Must be QDDP eligible now or in the future.
