Summary:

The RN Coordinator provides patient care coordination for home health patients. Educates patients, physicians and staff. Facilitates continuity of care of patients from hospital to home. Participates on an interdisciplinary team and collaborates with physicians and staff to establish and implement home care needs. This is an Inpatient role located primarily at UNC Medical Center with some travel to other UNC hospitals.

Responsibilities:

Patient Coordination & Care Navigation

  • Serve as primary clinical point of contact for patients referred to infusion therapy.
  • Conduct initial patient outreach to explain the infusion process, expectations, and care timeline.
  • Asses patient needs, potential barriers to care and coordinate appropriate resources
  • Ensure patients understand therapy requirements, pre-infusion instructions, and follow up care.
  • Work closely with intake team to assist in facilitating a smooth transition between inpatient and home infusion services. 

Clinical Assessment & Triage

  • Work closely with Case Managers and medical team to determine infusion readiness.
  • Identify issues that may delay therapy ( e.g., abnormal labs, missing orders, medication clarification, caregiver support)
  • Communicate findings with prescribers, pharmacists, and the infusion team promptly.
  • Provide clinical judgement to escalate complex patients to Clinical leadership.

Referral Management

  • Monitor incoming infusion referrals for completeness and appropriateness.
  • Ensure required documentation (orders and/or Home Infusion referral), insurance verification, and authorization is completed.
  • Prioritize referrals based on acuity, therapy type, and scheduling capacity.

Interdisciplinary Collaboration

  • Partner closely with pharmacists, infusion RNs, scheduling, and care teams.
  • Participate in daily/weekly interdisciplinary huddles to review patient flow and barriers.
  • Communicates therapy changes, delays, or clinical concerns to team members in real time.
  • Support escalation of operational or clinical issues affecting patient throughput.

Patient Education & Support

  • Provide disease-specific and medication-specific education related to infusion therapies
  • Ability to provide coverage of infusion referrals at other UNC facilities.
  • Ensure patients receive and understand teaching on side effects, symptom monitoring and when to seek help.
  • Provide psychosocial and emotional support, referring to social work as needed.
  • Document clinical education in EMR for TJC compliance.

Education Requirements:
● Graduation from a state-accredited school of professional nursing. BSN preferred.
● If hired after October 1, 2015 must be enrolled in an accredited program within four years of employment, and obtain a Bachelor's degree with a major in Nursing or a Master's degree with a major in Nursing within seven years of employment date.
Licensure/Certification Requirements:
● Licensed to practice as a Registered Nurse in the state of North Carolina.
Professional Experience Requirements:
● Two (2) years of home health, public health or other health related experience.
