QuadMed is looking for a LPN to join our team in Sheboygan, WI. Join our employer health center with a focus in primary care!

Schedule:Full-time

GENERAL PURPOSE OF JOB

The LPN is part of the teamdedicated toprovidingcomprehensive, accessible care, and superior evidence-based medicine through innovative practices focusing on wellness and prevention. The LPNwill be a highly motivated, quality-conscious healthcare professional with the ability to work in an environment where one-on-one time with patients and positive outcomes are the ultimate measurement of success. The successful candidate willfocus on prevention and wellness, perform all nursing functions, phone triage, coordinate care with external providers, and patient education as directed by the providers on site.

KEY RESPONSIBILITIES

  • Continuously provides quality care to all patients. Treats all patients with dignity and respect. Understands, follows, andmaintainsall Patient Rights.
  • Ensuresaccurateandtimelydocumentation into the EMR for all patient care;i.e.-patientinterviewsand history, vital signs, treatments, test results.
  • Maintains confidentiality of all patient and organization information. Follows federal and state regulations and organization policiesregardingprotection of patient information.
  • Assistsin the modification of patient treatment plans asindicatedby patients' responses, conditions, and as ordered by the provider.
  • Consultsand coordinateswith internal and external partners to plan patients' health activities, treatment, and referrals.
  • Prepares and administers medications and immunizations per provider'sorder.
  • Preparespatients for andassistswith examinations and treatments.
  • Performsdiagnostic testing toassistthe provider inidentifyingand assessing patients'conditions.
  • Cleansand sterilizesmedical instruments and equipment.
  • Ensurerooms are stocked with supplies.
  • Performsadministrativefunctions, front officefunctionsand schedulesappointments asrequested.
  • Referspatients to specialistswithin their elected health plan or community agencies.
  • Participates in professional development activities andmaintainsprofessional affiliations
  • Performs other position related duties as assigned
  • Certain clients and health centers mayrequiretravel

JOB REQUIREMENTS

Education:

  • Graduate from an approved nursing program with an associate degree or equivalent

Experience:

  • Minimum one (1) year of clinical experience

Certificates, Licenses, Registrations:

  • Current license in state of residence or compact licensure
  • Current CPR (BLS) certification or must be obtained within60 daysof hire
  • Phlebotomy certification preferred

Knowledge, Skills & Abilities:

  • Must have working knowledge of practical nursing theories and polices
  • Requires the ability to communicate effectively both orally and in writing
  • Requires organizational anddecision-makingabilities
  • Requires excellent assessment skills
  • Computer skillsrequired(electronic medical record experience preferred)
  • Bilingual in Spanish a plus, but notrequired
  • Proven ability to work effectively with diverse populations and a demonstrated commitment to fostering inclusion.
